You will be required to submit the documentation listed below for each action you are reporting. Please be aware your Housing Specialist may contact you if additional documentation is needed.

  • Personal Declaration
  • Income Verification
  • Bank Statements/Assets (Required only if value is greater than $5,000)
  • Tax Return (Self-employed individuals only)
  • Verification of Child Care Expenses
  • Verification of Student Status
  • Verification of Medical Expenses (Only if Head of Household or Spouse is Elderly or Disabled)

  • Zero Income Reporting Form (if applicable)
  • Income Verification (if applicable)

  • Personal Declaration
  • Notice to Vacate signed by yourself and your Landlord
  • Set an appointment to meet with your Housing Specialist (Not required, but highly recommended)

Change in People in Household

  • Personal Declaration
  • Landlord Approval to add the individual to your lease
  • (214) Declaration of Citizenship Form
  • Social Security Card
  • Legal Custody Documents (If applicable)
